DATA SUBJECT APPLICATION FORM

Under the Law on the Protection of Personal Data No. 6698 (“KVKK”), the data subject, defined as the individual whose personal data are processed (“Applicant”), is entitled to submit a request to the data controller regarding their rights set forth in Article 11 of the Law.

Pursuant to Article 13, paragraph 1 of the KVKK, data subjects must submit their requests regarding their rights in writing to our Company, which acts as the data controller, or by other methods determined by the Personal Data Protection Board (“Board”).

Within this framework, applications to our Company must be submitted in writing by printing out this form and delivering it through one of the following channels:

• By the Applicant in person,
  • By post or through a notary public,
  • By sending it to our Company’s registered electronic mail (KEP) address, duly signed with a “secure electronic signature” as defined in the Electronic Signature Law No. 5070.

If the Board determines any additional methods other than those specified above, our Company will announce how applications can be made through those methods.

Applications submitted to us will be evaluated and responded to within 30 (thirty) days from the date of receipt, in accordance with Article 13(2) of the KVKK. Responses will be sent to you in written or electronic form as stipulated by the same Article.

To prevent any legal risks that may arise from unlawful or unjust data sharing and to ensure the security of your personal data, our Company reserves the right to request additional documents and information (such as a copy of an ID card or driver’s license) to verify your identity and authority.

Please note that our Company will not be held legally liable in cases where the information or documents provided in this form are inaccurate, outdated, or where an unauthorized or unlawful application or request has been made.
